Duane Reade by Walgreens offers the customers, who have recently visited a Duane Reade location and received a specially marked purchase receipt, to complete the Duane Reade Customer Satisfaction Survey and share their opinion about the products and service they received at the store. To value their opinion and say thanks for taking the survey, all eligible entrants will be given the opportunity to enter the sweepstakes to win the prize of the contest upon completion of the survey.

Duane Reade Survey Sweepstakes Rules & Requirements
To participate in the Duane Reade Customer Satisfaction Survey Sweepstakes, you must comply with the contest rules and requirements such as
- Must be a legal US resident excluding Puerto Rico
- Must be at least 18 years of age or older at the time of sweepstakes entry.
- Make an eligible purchase at a participating Duane Reade store and receive a sales register receipt containing the survey invitation and survey code and password printed on it.
- The survey must be taken within 72 hours upon receipt of the invitation.
- Must not be the employees of the Duane Reade and Walgreens family of companies or any of their associated companies and their immediate family members or any individual involved in the program.

Several ways there are to enter the Duane Reade Customer Satisfaction Survey Drawing Contest. Here is a brief description of each of them to help you enter the sweepstakes rightly. Choose any of them at your convenience and as per the availability.
Online Entry
- Visit the Customer Satisfaction Survey site on your Duane Reade receipt or go to www.drelistens.com
- Enter the survey code, password, and select the time of visit from your Duane Reade receipt. (View the sample receipt image posted next to the entry fields indicating the locations of the required information on the receipt).
- Click on the “Start” button to begin the survey.
- Fill out the survey to the best of your knowledge.
- Once you complete the survey, submit the required details and accept the official sweepstakes rules to receive one entry into the sweepstakes.
Phone Entry
The eligible participants, who are not willing to take the Duane Reade customer survey online, can do it over the telephone via an automated customer satisfaction survey. For that, you can call the number on your survey invitation or 1-800-219-7451 and respond to the recorded prompts. At the conclusion, you will have an opportunity to provide your Sweepstakes Entry information for the applicable sweepstakes entry period.

Mail-In Entry
To participate also in the Duane Reade Customer Satisfaction Survey Sweepstakes through Mail-In entry without making any purchase or taking a survey, you will need to take a 3” × 5” piece of paper and legibly hand-print your name, age, complete address, daytime and evening phone numbers, and date of birth (MM/DD/YYYY) and send it in an envelope to the following mailing address.

Selected Duane Reade Customers, who have agreed to accept email messages from Duane Reade may receive a survey invitation emailed to them. Click the link on the email invitation and follow the instructions you will be provided to enter the related Sweepstakes Period.
A Few Important Notes on Duane Reade Survey and Sweepstakes
- The winners will be selected in a random drawing from among all eligible entries received.
- Each potential winner will be contacted by phone approx. 14 days after the drawing date to verify their mailing address.
- Each of them will then be notified by mail approximately 7 days after verification of the mailing address.
- An alternate winner will be selected if prize notification or prize returns as undelivered or failure to attempts to contact the winner or failure to return completed and signed affidavit and release within 7 days.
