• MyBMGChart Activation Code.
• MyBMGChart Username.
• MyBMGChart Password.

To obtain credentials, you need first and enroll and register in MyBMGChart. Got through the following steps to access MyBMGChart facilities;

Step 1

Request for MyBMGChart Activation Code:

• Open the browser and visit www.mybmgchart.com/mychart
• At the right corner, click on the ‘Sign up Now’.

• Click on the ‘No Access Code? or directly go to this link.

• You will be re-directed to a page, where you need to enter the following information to request access code.

1. Full name (First, Middle and last name as per the records).
2. Residential address. (Need to select the country, state and zip code from the dropdown.)
3. Date of birth
4. Gender.
5. Last four digit of Social Security Number.
6. Residential contact number.
7. Email address. (Please provide an active email address while you access frequently).
8. Verify the email address.

• On entering the information, type the CAPTCHA code given.
• Click to confirm.
• Click on the ‘Submit’ button.

Your information will be sent to an administrator for verification. The verification process takes one week to complete. On approval, you will receive the Account Activation Code via email and letter to your email address and residential address respectively.

Step 2

Enrolling for MyBMGChart for Username and Password:

Once you receive the Activation Code via email, start following these steps to get registered quickly.

• Open the browser and visit www.mybmgchart.com/mychart
• At the right corner, click on the ‘Sign Up Now’.
• You will be re-directed to a registration form where you need to enter the following information:

1. MyBMG Chart activation code. (This code is only for the registration. No need to use it after complete enrolment.)
2. Last four digits of your Social Security Number.
3. Date of birth. (In mm-dd-yyyy format).

• After entering all the info, click on the ‘Next’ button.
• In the next step, you need to create your USERNAME and PASSWORD. These credentials are your gateway to the portal. So enter them very carefully and correctly.
• Click on the ‘Submit’ button.

You will receive the confirmation of your registration on your email. You are now ready to log-in to your MyBMGChart account and avail the services.

Step 3

Steps to log-in MyBMGChart account:

• Open the browser and visit www.mybmgchart.com/mychart (mark it as a bookmark for future access)
• Go the right column and enter the Username and Password in the respective field.

• Click on the ‘Sign-In’ button.
