Rules to Receive the Home Depot Survey Rewards:

Each potential Prize Winner must complete, sign and return a notarized “Affidavit of Eligibility/Liability/Publicity Release Form” (the “Release”) and an IRS Form W-9 (collectively, the “Required Documents”), within the date and time indicated within the Notification.

If a potential Prize Winner fails to return the Required Documents within the date and time frame indicated, the potential Prize Winner forfeits his/her right to the respective Prize and another Prize Winner will be selected at random from among the remaining non-winning eligible entries received within the respective Category.

The Sponsor (or a party acting on its behalf) must report the value of the Prize to the IRS in the year the Prize is received by the Prize Winner and will be reported to the Prize Winner and the IRS in the form of a Form 1099.

The Prizes will be awarded within six to eight weeks from the date on which the Required Documents are received.
